Asuhan Kebidanan Komunitas pada Ibu Nifas dengan Perawatan Luka Perineum: Laporan Kasus Tiga Kunjungan di Wilayah Kerja Puskesmas Mantang, Kabupaten Lombok Tengah

Abstrak

Background: Perineal wounds from laceration or episiotomy are common during vaginal birth, and inadequate wound care in the postpartum period remains an important contributor to puerperal infection and maternal morbidity in Indonesia. Case-level documentation of community midwifery management of perineal wound care across sequential postpartum visits remains limited compared with population-level studies. Objective: This case report describes community midwifery care using the Subjective-Objective-Analysis-Planning (SOAP) framework for a primiparous woman with a second-degree perineal wound at Mantang Primary Health Care Center, Central Lombok Regency, followed across three home visits. Method: A descriptive case-study design was used. Data were collected through interview, physical and obstetric examination, and vital-sign measurement on postpartum day 1, day 3, and day 8. Results: The client, a 30-year-old primipara (P1A0H1) who delivered spontaneously with a second-degree perineal laceration, presented on day 1 with pain at the suture site and a moist wound; vital signs were normal and lochia was rubra. She was diagnosed with first-day postpartum with perineal wound and was managed through wound hygiene education, early mobilization, a high-protein diet, and collaborative analgesic and antibiotic therapy. By day 3, pain had decreased with lochia progressing to serosa, and by day 8 the wound had fully dried with lochia alba and no signs of infection. Conclusion: This case illustrates how a holistic, evidence-informed SOAP-based midwifery approach delivered across scheduled postpartum visits can support uncomplicated perineal wound healing and early detection of infection risk at the primary-care level. The findings support strengthening structured postpartum visit schedules and perineal hygiene counselling as complementary strategies to reduce puerperal infection in community midwifery practice.
